Quality Care by Compassionate Hearts

  • Hospice care specializes in care for those facing a life limiting illness. But it is more than just caring for a persons physical needs, the social and emotional care given to a person is just as important or even more so. Our care is done at the patient's home or a home like facility. Our certified staff is able to provide a patient and their families with the care they need wherever they choose and whenever it is required.

  • Patients that live in a nursing facility or other long term care facilities can also receive specialized visits from physicians, nurses, and social workers alongside any other care and services provided by the facility.

  • Here at In Home Care Hospice, patients and their loved ones are cared for by a team of certified doctors, nurses, social workers and therapists working together in a team to address the patient's and family's needs. We help provide medication, equipment, personnel, or anything else required. Hospice care is fully covered by most major insurance companies. It is provided both by Medicare nationwide and by Medicaid in most states. Most private health insurance policies also cover hospice care. To be sure of coverage, families should check with their health insurance provider.

  • Choosing a Hospice Care Provider is no easy task. One of the best ways to choose is to communicate with the Hospice Care Provider. Here at In Home Care Hospice, we are more than happy to answer any questions and help you understand our services and how we might help you and your loved ones. Patients have different needs, as well as their families and knowing what to ask us will put your mind at ease on how we take care of our patients and their loved ones. Give us a call at 1-818-875-4277 and see what we can do for you and your family.

The team manages overall patient care and perform periodic in home visits. Together, they develop a comprehensive care plan that is patient-specific, that addresses the medical, physical, emotional, and spiritual needs of patients and their family members.
